Deaf from birth, Piero Italiani began diving at 12-years-old. At the 1978 European Junior Championships in Firenze, he won the platform title and was runner-up on springboard. After a bronze on springboard at the 1983 Mediterranean Games, he competed at his first Olympics in Los Angeles, placing sixth on springboard, the best finish by an Italian diver since Klaus Dibiasi and Franco Cagnotto.
After winning two medals at the 1987 Mediterranean Games (gold on springboard and silver on platform), Italiani competed at the 1988 Seoul Olympics where failed to advance to the springboard final. After retiring he worked at the Italian Swimming Federation.
