Medhat Youssef’s basketball career began at the Young Men’s Muslim Association and took off when he began to represented Egypt internationally after World War II. He was part of the national team that travelled to the 1948 London Olympics, where it placed 19th overall in a field of 23 nations. He had much better luck at the 1949 European Championships and the 1951 Mediterranean Games, as Egypt won the gold medal at both events. In-between, he helped Egypt finish fifth at the World Championships in 1950. He was also on the squads that were eliminated after round one at the 1952 Helsinki Olympics and placed eighth at the 1953 European Championships. Following his retirement from active competition, he worked in the administration of the sport, eventually becoming the president of the national federation briefly in 1988, following the death of his long-time teammate Youssef Abou Ouf.
