Australia and the United States were expected to extend their women’s swimming relay rivalry in this event. Although Australia won both women’s freestyle relays, the USA depth in all strokes likely made them a slight favorite in the medley relay.

Australia led the qualifying round, followed by Canada and China, with the USA in fourth, although the Aussies and Americans swam B teams, and would replace their entire squads for the final.

In the final, Regan Smith put the USA in first on the opening backstroke leg, with an Olympic record 52.28, ahead of Kaylee McKeown, who had beaten Smith in the 100 back in Paris, and the race was never close. The Americans extended their lead on the next two legs, with Lilly King’s breaststroke putting them over 2½ seconds ahead and, although Mollie O’Callaghan closed slightly on Torri Huske on the anchor, the USA won in a world record 3:49.63, 3.48 seconds ahead of Australia. China had been second at the final exchange but Yang Junxuan could not match O’Callaghan on the anchor leg.

McKeown’s silver medal gave her 14 Olympic medals, tied for the most by any woman ever, equaling Katie Ledecky, who also won her 14th medal in Paris. O’Callaghan’s won her eighth Olympic medal, seven of them coming in relays at Tokyo and Paris, along with 200 free gold in 2024.
