China’s Xu Jiayu and Italy’s Thomas Ceccon were the favorites. Xu had the year’s fastest time and was a two-time World Champion in the event, while Ceccon was second on the world list for the year and was the 2022 World Champion.

Both easily made the final, with Xu first in the semi-finals and Ceccon second, along with 2016 gold medalist Ryan Murphy, who had also taken bronze at Tokyo 2020. In the final, Xu went out very fast, leading at 50 metres by 0.16 seconds over Murphy, with Ceccon in third. But Xu had the slowest split of the eight finalists in the final 50 metres and would have to hold on for a silver medal. Ceccon had the fastest finish to take the gold medal, while Murphy also finished slowly, but held on for his third consecutive medal in the event.

Ceccon’s gold was the first medal ever for Italy in the men’s 100 backstroke. Murphy’s three medals in the event had only been done previously by East German Roland Matthes, who won gold in 1968 and 1972, and bronze in 1976.
